Each question will generate a score which we will look at in our next catch up and use to track how you're doing

To get the best out of the questionnaire

  • Chose the answer that best characterises you - your honesty will establish the best approach for you

  • If there are questions you feel uncomfortable answering, please do not worry - some of them can be difficult, we can come back to them at another point and can talk about them whenever you feel happy to

  • Answer questions in the here and now - please don't base answers on how you felt a couple of weeks ago or how you hope to feel in a few weeks' time

  • Try not to over think answers - go with your first instinct rather than taking time to ponder
