Client Agreement Form

In order for My Happy Balance to be a successful experience for you, we need to know what to expect from each other. This agreement will help both you and I know what our role is, and what we can expect of each other so that we can work together with confidence and openness.


Please read through the form and check the tick box if you agree. If there is something you’d like to discuss please do let me know as all terms are negotiable within reason and I want you to be happy and safe before we proceed. Thank you.

As a member of My Happy Balance you agree to...

  • Give at 24 hours’ notice if you are unable to attend any pre-agreed calls

  • Be open to considering new ideas and habits that suit you and your goals

  • To give feedback to your sessions

  • To let me know of any injuries, niggles or illness

  • To be honest and open with your progress, barriers, actions taken etc.

  • To let me know if you have any questions, difficulties or worries

  • To fill out the PAR-Q honestly and to the best of your ability, and to let me know if anything changes

  • To Update your payment subscription if your payment details change

In return, as owner of My Happy Balance, I will...

  • Celebrate the great work you put in

  • Work through any barriers, difficulties or questions you may have to the best of my abilities

  • Come back to you in a timely fashion with feedback and answers to any questions you may have

  • Be open, honest, and professional at all times

  • Be supportive to you throughout

  • To offer feedback to your progress

  • Return all client documents on time

  • Attend all pre-agreed meetings or sessions, providing at 48 hours’ notice if I cannot attend

  • Keep your data secure, and delete if you ask me to

  • Maintain full client confidentiality
